How/where do I submit the waiver once I have completed the training program?

You can print the waiver form and mail it in or you can submit it electronically. Visit Buprenorphine.SAMHSA.gov to find the print and online version of the form. If you submit your form online, you do not need to submit documentation regarding your training. We submit to it SAMHSA bi-weekly.
