Step 3: Complete the Demographics Profile
Provide some important details about yourself to help create your Continuing Education Certificates or Letters of Completion.
Step 4: Complete the Program (Pre-Assessments, Modules, Post-Assessments)
The successful completion of the program requires pre-assessments, interactive modules, and post-assessments. Post-tests require a 70% passing rate.

Step 6: Print your DATA 2000 Training Certificate and Submit Your Waiver Request
Your DATA 2000 Training Certificate will be available once you have completed the coursework and requested credit.
If submitting the Waiver Notification Form form by mail or fax, you must also send a copy of your DATA 2000 Training Certificate along
with the waiver form. If you complete the online version of the waiver form, it is not necessary that you mail or fax a copy of your

Step 7: Complete Follow-Up Surveys
You will receive an email invitation to complete optional follow-up surveys to assess practice change and patient outcomes.
Step 8: Expand Your Knowledge and Promote Your Practice
